PUPPY QUESTIONNAIRE

Thank you for your interest in adopting a puppy from us!

This application is to get to know our potential families better to ensure that you get the best friend you're looking for, and the puppy gets the best home that it deserves.

These questions also serve as a great way to get you thinking about all that comes with getting a puppy. Each question is very important for you to answer honestly. Please read all of the following carefully and complete each section of the form.

Our puppies get daily interaction with us, our children, and our other dogs. All of our parents are tested through PawPrint Genetics DNA, OFA. Puppies are dewormed at 2, 4, 6, and 8 weeks old. All puppies will get health checked from our vet and up to date on all shots at 8 weeks old. All puppies go home with a care package to help them ease into their new homes. Along with lifetime Breeder support from us. Throughout their 8 weeks with us, they will get to experience many different sounds, textures, and of course lots of love. We love to send our new puppy owners many pictures and videos so that around week 6, you can make your selection of which puppy you prefer. Puppy picks are based on order of deposits and are conducted via FaceTime ONLY! There are no home visits until it is time to pick up your puppy at 8 weeks old, absolutely zero exceptions. Please do not ask.

Adoption fee is $2,000. There is a non refundable deposit of $300 due when there is confirmation of pregnancy.

We only offer limited AKC registration, no breeding rights except to preapproved and reputable breeders.

Our contract does require a spay/neuter agreement.

Deposits are not accepted until application has been reviewed and approved. We accept 6 pre- deposits per waiting list, 3 of each gender. In the event that not enough of each gender is born, you would have the choice of getting the opposite gender or transferring your deposit to the next waiting list. If your application is not selected before puppies are born, it very well could be after puppies are born if we have more born than expected. Please know that nothing is guaranteed.

Once we receive your application, it will be thoroughly reviewed. If it is approved, you will be emailed more information about our expected litters and waiting lists. Once we have agreed upon your spot on our waitlist, we will also send you a copy of our contract and deposit form to be completed. Only approved applications wil be contacted. After the litter is born, we will approve more applications if more puppies are available.Please feel free to contact us for further information or questions.

Do you understand then financial responsibility of a dog with expected and unexpected vet visits? Our dogs are tested as clear of all genetic disorders and diseases, however things still can come up that cannot be tested for or prevented. Examples: cancer

Do you understand that submitting this application does not mean that you are automatically approved and placed on our waitlist? We carefully chose each family for our puppies and strive to match you with a puppy that will fit your family and lifestyle.

Please do not submit this application with no intention to reply to our messages. We put a lot of effort in going through each application to not hear back from our applicants. If you change your mind, that’s totally fine and understandable. Just let us know that when we contact you. Thank you!
